Statement:
Comprehensive Geriatric Care
is a group of geriatric physicians and other health care
professionals working together to provide health care to older
adults in their home, our offices, hospitals, and nursing homes. We
aim to develop innovative solutions to ease transitions in care
settings.
Our integrated team of
medical providers share a common vision about the quality of care
and we strive to meet the standard of the best medical
care.
We understand that the aging
process results in a spectrum of health care requirements for
individuals within a community. Some older adults continue to have excellent health and
independent functioning and therefore warrant an approach focused on
health maintenance and health care planning. Other older
adults with a variety of interacting medical conditions, are
dependent on others to meet their needs in daily activities, and
require frequent evaluation of medical and psychiatric symptoms,
coordination of medications, timely specialty consultation and a
palliative approach.
Our services and programs focus on providing health care
which has the ability to meet the needs of the full spectrum of
health care goals which occur with
