The
mission of the home visiting
program is to provide coordinated interdisciplinary
health care for older adults who are homebound.
Our
services integrate transitions
to home-based health care for older adults who are no
longer able to make office visits, or who are
leaving
the hospital or nursing home.
Target Patients
We serve older adults who are homebound because
their illnesses prevent practical transportation to a
medical office. These illnesses may
include:
Advanced neurological disease including
dementia
Advanced pulmonary
diseases
Advanced cardiac disease
Severe musculoskeletal
disease
Severe
psychiatric conditions
Complex wounds
Terminal illness / needing palliative
care
Referrals
Home Visiting Program referrals are accepted from
older adults, hospitals, nursing homes, elder service
agencies, visiting nurses, friends and
neighbors.
