Overview
Elder Care Planning is an elder focused, family-based
assessment of the bio-psychosocial needs and resources of an older
adult resulting in written recommendations designed to meet the
individual goals of the elder and family through referral to the
appropriate resources including caregiver agencies, legal services,
support groups and living facilities.
Services
Elder care planning consists of a 90 minute in-person session
with the elder, involved family members or direct caregivers and a
licensed certified social worker. The sessions can be conducted in a
single session or divided into one 60 minute session and one 30
minute session. The sessions are conducted in the elder’s home or in
Comprehensive Geriatric Care’s office. The purpose of the session is
to conduct an assessment of: the elder’s medical and psychiatric
history, functional abilities, social support system, financial
resources; outstanding needs of the elder and the family or
caregiver; safety concerns; the elder’s future life goals; and the
goals of the family or caregiver.
Within one week of the session, a written report will be
provided to the party seeking elder care planning. The report will
include the intake assessment, a list of recommendations including a
list of individualized resource referrals appropriate for
instituting a care plan.
Referrals may include the following: Medicaid and other
entitlement programs, home care agencies, nursing homes, assisted
living facilities, hospice care, caregiver services including
support groups, respite care, and individual and family counseling.
Services are provided by Franca Martino-Starvaggi, a licensed,
certified social worker with 15 years of experience in the Bronx.
Following the initial assessment, Comprehensive Geriatric
Care, PC will provide phone consultation and liaison services for
the resource referrals for a period of thirty days. Services
can be extended through Comprehensive Geriatric Care’s Care
Management program
Medicare and Insurance Coverage
Elder Care Planning is a non-covered service under
Medicare and all other health insurance
programs.
