Overview

As an extension of its Elder Care Planning services, Comprehensive Geriatric Care offers Care Management services. Care Management is continuous oversight of the care plan of an elder which focuses on quality of life.  Care management is designed to meet the individual goals of the elder and family through ongoing re-evaluation of the goals of care as needs and resources change over time.

Services

The Care Management program is an optional continuation of services following Comprehensive Geriatric Care’s Elder Care Planning services program.  Care management services consist of the continued coordination, monitoring and evaluation of needs and goals of both the elder and their family or direct caregiver. The purpose of Care Management program is to provide resources that are needed by elders and their families to navigate the healthcare system. Services include conducting in-person or phone meetings with the elder and their family to further assess their needs and goals, and acting as a liaison with health care agencies, health care providers, insurance companies, government health and entitlement entities on behalf of the elder and their family. Services are provided by Franca Martino-Starvaggi, a licensed, certified social worker with 15 years of experience in the Bronx.

Medicare and Insurance Coverage

Elder Care Planning and Care Management are currently non-covered services under Medicare and all other health insurance programs.
